Registration Form and Census Information

It is important that you register in the parish since weddings, baptisms, confirmations and other parish services require registration. Also, if you are asked to be a Godparent, Confirmation Sponsor, or a Witness for a Wedding, your pastor must sign a form stating that you are a practicing Catholic and attend Mass regularly. If you are not registered, he cannot sign this form.

Registration forms can be downloaded in pdf format. They can also be found in the pamphlet racks in the vestibule of the church.

There are three easy ways to send the information:

1. Drop it in the collection basket.

2. Mail it to us at:
